An application form will need to be completed and submitted to the Application Review Committee. Interested families may call the Miami County Habitat office at (937)332-3763 to sign up to receive an application. Once the family's name is on the list an application will automatically be mailed to their address when applications are being accepted. Office hours are Monday-Friday 9:00am-5:00pm.

A letter will accompany the application with further information for completing and submitting the form.
